Team-Based Care Toolkit for Integrated Primary Care
The Team-Based Care Toolkit for Integrated Primary Care is a guide to support Federally Qualified Health Centers (FQHCs) in the move towards Value-Based Care. C3’s Integrated Primary Care Framework forms the foundation of this toolkit. Integrated Primary Care (IPC) is a new vision for the financing and delivery of primary care for FQHCs.

This toolkit consists of the following sections, which align with the key domains of the IPC Model:
